Float Application Forms and Waivers
All applications, liability waivers and
narrations must be submitted to the Float committee no later than March
30, 2008 or
as soon as possible, for the confirmation and assignment of
island. APPLICATION: Click on the link below for the application form in PDF format. WAIVERS: KKDCP waiver forms must be submitted with each application; Click on the Link below to download PDF files. Float participant liability waiver .All participants must complete and sign waivers. Minors ( 18 and below ) must have their parents or guardian sign a waiver prior to any participation.
